Bryan (Austin Abrams), a young organ courier, accidentally runs over a mysterious woman while making a nighttime delivery to Raccoon City Hospital.
What initially appears to be an ordinary accident instead triggers a chain of uncontrollable events, destined to turn the night into a nightmare and plunge the entire city into panic. Within a few hours, a mysterious epidemic begins to spread, altering human DNA and giving rise to monstrous creatures born from terrifying bioengineered experiments.
Trapped in a Raccoon City that has descended into chaos, Bryan will have to make his way through violence-ravaged streets, face hordes of mutated creatures and forge unlikely alliances in order to stay alive.
As time relentlessly runs out, he will try to uncover the origin of the infection and stop a threat that risks wiping out any chance of salvation for the city and its inhabitants.
